You will need some basic forms to register your patients and
caregivers. The forms do not have to be elaborate. Some samples are below:
The Patient Registration Form is used to record the personal
information on the patient.
The Client Code of Conduct defines the rules for how your clients
must conduct themselves.
The Hold Harmless Agreement is a legal document stating that the
client holds you harmless for various acts. In other words, if they get hurt
they won't sue you. Hold harmless agreements are not always enforceable because
you can't sign away your legal liabilities. It depends on the conditions. But
they don't hurt.
In addition, you will want specific written procedures for how
clients are registered. In business you always want the procedures written out
in detail, even if the procedures would seem obvious. It helps to eliminate
confusion, and it will help greatly in training new employees. At a minimum, it
should include something like:

Verify that all forms are completed properly.

Copy or scan patient ID and recommendation form.

Contact the doctor to verify that the recommendation is valid.

Issue patient ID or first-time customer coupons, as appropriate.

File the paperwork in the appropriate place.
